Alternatives to laser eye surgery: What is possible when laser eye surgery reaches its limits

Laser eye surgery is not always possible – but that is no reason to worry. If you are not suitable for LASIK or ReLEx SMILE, there are strong alternatives today: for example, modern lens implants such as the EVO Visian ICL or lens replacement with multifocal lenses. In this article, we show you when laser eye surgery reaches its limits – and what options are then open to you.

When does an alternative to laser eye surgery make sense?

Laser eye surgery is a proven and gentle method of permanently correcting defective vision. Modern procedures such as Femto-LASIK, ReLEx SMILE or PRK enable precise results – often even completely pain-free. However, not everyone is suitable for laser eye treatment. In certain cases, these procedures reach their medical limits.

Limits of modern laser eye procedures such as LASIK or ReLEx SMILE

Whether LASIK, LASEK or ReLEx SMILE, all procedures require the cornea to be thick enough to remove part of the corneal tissue with the excimer laser during the procedure. In the case of a thin cornea, certain degrees of corneal curvature or an unstable corneal structure (e.g. keratoconus), laser eye surgery is often not an option.

Laser refractive surgery also reaches its limits with very severe short-sightedness, long-sightedness or pronounced presbyopia. The same applies to people with chronically dry eyes or after certain eye operations such as cataract surgery.

In such cases, an alternative to laser eye surgery may be more sensible and safer – for example, implantable contact lenses (ICL) or lens replacement with modern artificial lenses.

Lens surgery: a precise alternative to laser eye surgery

If laser eye procedures such as LASIK or ReLEx SMILE are not medically possible or not desired, modern lens surgery offers a safe and effective alternative. It is particularly suitable for severe ametropia, thin corneas or if reversible correction (at least with ICL) is preferred.

What happens during lens replacement?

Lens replacement involves replacing the natural lens of the eye with a customized artificial lens – similar to cataract surgery. This procedure is particularly suitable for presbyopia, severe farsightedness or if the patient’s own lens is already losing its transparency due to age. Modern multifocal lenses caneven compensate for several visual acuities at the same time – e.g. both near and distance vision.

The procedure is performed under local anesthesia with eye drops, is usually painless and is performed individually for each eye. Full visual acuity is usually achieved just a few days after the operation. Follow-up checks ensure that the eye adapts optimally to the new lens.

EVO Vision ICL – the implantable contact lens for clear vision

The EVO Visian ICL is a particularly elegant solution for people with severe short-sightedness, long-sightedness or astigmatism for whom laser eye surgery is not an option. The ICL is implanted as an additional lens between the iris and the natural lens – without removing any tissue from the corneal surface.

Advantage: ICL lenses are reversible – they can be removed or replaced if necessary. In addition, the natural lens of the eye is preserved, which makes this procedure a particularly gentle alternative to laser eye surgery. Many patients report an impressive improvement in visual acuity – often even better than with glasses or contact lenses.

Differences between lens implantation and laser eye surgery

The main differences lie in the principle of the treatment method:
While corneal tissue is removed during laser eye surgery (e.g. with FemtoLaser), the structure of the eye is completely preserved during lens implantation. This makes procedures such as the EVO ICL particularly suitable for sensitive corneas, dry eyes or very high ametropia.

Lens implantation is also often an option if laser treatment cannot be carried out for medical reasons – for example after previous eye surgery, in the case of unstable corneal epithelium or as a supplement to classic refractive surgery.

For whom is lens surgery more suitable than LASIK or ReLEx SMILE?

Lens implantation or lens exchange may be the better choice if:

  • the cornea is too thin for safe laser treatment

  • very high myopia or hyperopia is present

  • there is also a complex degree of astigmatism

  • the corneal epithelium is unstable or pre-damaged

  • chronically dry eyes make laser eye surgery more difficult

  • Presbyopia or incipient cataracts are present

  • a reversible solution is preferred (e.g. with ICL lenses)

  • previous eye surgery or complications on the corneal surface have already been documented

  • the goal is a particularly precise correction with long-term visual acuity

Conventional vision correction: when the laser is not (yet) an option

Not everyone wants to opt for laser eye surgery or lens implantation straight away. Especially at a young age or with unstable vision, traditional visual aids such as glasses or contact lenses are often the first choice – and for good reason.

Glasses or contact lenses – proven methods with limitations

Spectacles are uncomplicated, low-risk and easy to adjust. They are particularly suitable for everyday wear and the office, for low to moderate visual defects. However, they do have limitations: glasses can be distracting during sporting activities, in changing light conditions or when working on a PC.

Contact lenses offer more freedom of movement and a wider field of vision. But there are limits here too:

  • Dry or sensitive eyes often react irritably to prolonged wear

  • Hygiene is crucial – otherwise there is a risk of infection or inflammation

  • Lenses are not suitable for every corneal shape or eye condition

  • They do not correct presbyopia or advanced lens opacities

Even though glasses or contact lenses can be a reliable solution, they are not a permanent substitute for medical correction – neither by laser nor by lens.
